Rule 292. Form of Summons in Proceedings to Review Orders of the Illinois Workers' Compensation Commission

Upon the filing of a written request to commence a proceeding to review an order of the Illinois Workers' Compensation Commission under either the Workers' Compensation Act, approved July 9, 1951, as amended, [FN1] or the Workers' Occupational Diseases Act, approved July 9, 1951, as amended, [FN2] the clerk of the circuit court shall issue a summons in substantially the following form to the Commission and all other parties in interest:
