§ 991. Summons to show cause; affidavit
AFFIDAVIT TO ACCOMPANY SUMMONS. The summons must be accompanied by an affidavit of the plaintiff, his agent,
representative, or attorney, that the judgment, or some part thereof, remains unsatisfied, and must specify the amount due thereon.
